1. This offer is made by Corning Life Sciences (“CLS”), a division of Corning Incorporated.
2. The Product(s) eligible for this offer and details of the applicable guarantee period are published on (Substitute with short link we define later) (the “Products”). We reserve the right to withdraw the offer at any time.
3. This offer is open to individuals who have purchased an eligible Product directly from CLS and have used it in accordance with its operating instructions. If after using the product you are not satisfied with results, you may apply for money back in accordance with these terms and conditions if:
a. The Product has been used with the highest attention to aseptic laboratory practices.
b. Is thoroughly sanitized with 70% isopropyl ethanol before and after each use.
c. Can be claimed as non-hazardous by CLS upon receipt thereof.
4. The guarantee period runs from the date of delivery until [30] calendar days thereafter.

Contact (CLScustserv@corning.com / 800 492 1110 Option 2) to initiate return together with:
a. The original receipt, as proof of purchase; PO and unit serial numbers found on the packing slip
b. The completed and signed Decontamination Form and other required documentations
c. The product packaged securely in its original product box
6. The order will be refunded, for the purchase price of the Product paid, within a period of [30] days of receipt of the Product and completed documentation mentioned above.
a. The returned product must reach CLS by day 45 after initially receiving the order.
7. You may only make a maximum of 1 claim per Product.
8. No responsibility by CLS can be accepted for claims for Product lost or delayed in transit, or incomplete or illegible forms. Proof of shipment will not be accepted as proof of delivery.
9. All care should be taken to ensure the returned Product is adequately packaged in the original package and protected from transit damage to avoid loss or damage. The customer is responsible for any damage or loss of the Product prior to its delivery back to CLS. We reserve the right to deduct from the refund the cost of repairing any damage to or replacing the loss of the Product.
10. CLS will not accept any liability for additional costs associated with the return, proof of delivery or shipment of the Product.
11. This is a consumer offer only and neither retailers nor wholesalers may apply.
12. This offer cannot be used in conjunction with any other promotion run by CLS in connection with the purchase of the Product.
13. This offer applies to purchases and application use in North America only.
14. This offer can only be redeemed directly with CLS.
15. CLS shall not be liable for any delay or failure to perform due to any event beyond our control.
16. These terms and conditions will be interpreted in accordance with the laws of the State of New York.
17. Except as modified herein, CLS’ standard terms and conditions of purchase available here shall otherwise apply to your purchase of the Product from CLS.
